To process a diet you must present:

• Book for Food Products.
• Model 12 Sworn Declaration of Medical Diet, duly signed and stamped by the Public Health Assistance Center that issues it.
• The renewal of the medical diet may be processed by the consumer, any day of the month in which the current diet expires.
